Learn when to give infant acetaminophen for fever and pain, how to dose safely, and what signs to watch for. A practical guide for parents.

Quick answer

Give infant acetaminophen only when a doctor recommends it for fever above 100.4°F or for visible discomfort from teething, ear pain, or minor injuries. Always use the weight-based dose on the label.

Fever: when it helps and when it doesn't

A low-grade fever (under 100.4°F) is a normal immune response and usually doesn't need medicine. Infant acetaminophen is appropriate when fever reaches 100.4°F or higher and your baby seems uncomfortable, fussy, or is having trouble sleeping or feeding. It lowers fever but does not treat the underlying cause, so always monitor for other symptoms like rash, lethargy, or breathing trouble.

If your baby is younger than 3 months with any fever, call your pediatrician before giving any medicine. For older infants, acetaminophen can help bring a high fever down enough for rest, but it should not be given just to keep a number normal.

Pain: recognizing the signs in non-verbal babies

Babies can't tell you where it hurts, so watch for behavior changes. Common pain cues include persistent crying that's different from their normal fussiness, pulling at ears, rubbing the jaw or gums, difficulty nursing or taking a bottle, and waking more often than usual. Teething pain often peaks between 6 and 12 months and can be eased with acetaminophen if cold teethers or gum massage aren't enough.

Ear infections often cause nighttime crying and tugging at one ear. If you suspect an ear infection, acetaminophen can provide temporary relief, but you should still see a doctor for diagnosis. For minor bumps or bruises, acetaminophen can help if your baby seems unusually sensitive to touch or movement.

How to give infant acetaminophen safely

  1. Check the weight and choose the right product: Weigh your baby without clothes on a digital scale. Use the weight, not age, to find the dose on the label. Infant acetaminophen comes in a concentrated form (160 mg/5 mL) — never use children's or adult formulas. A common mistake is guessing the weight. Always weigh your baby first.
  2. Measure the exact dose with the syringe that came with the bottle: Shake the bottle well. Fill the oral syringe to the line that matches your baby's weight. Squirt the medicine slowly into the side of the cheek, not the back of the throat, to avoid gagging. Kitchen spoons are not accurate. Use only the syringe from the package.
  3. Wait at least 4 hours before the next dose: Do not give more than 5 doses in 24 hours. Write down the time of each dose so you don't accidentally double up. If fever or pain persists after 24 hours of acetaminophen, call your doctor. Setting a timer on your phone helps prevent missed or early doses.
